W-2’s were sent out January 31, 2008 from our central payroll department in PA.  They were sent to the address we have on file for you.  If we received a change of address from you before January 16, 2008, your W-2 will be sent to that address.  If you have moved and you did not supply us with a change of address, you will be able to pick up a duplicate W-2 or we can fax one to you.  You can request a duplicate any time after February 29 via email– info@crstaff.com.  Or you can fax your request to 770-813-8825.  Please include a fax number where we can fax your copy.  If you would like your copy mailed,  please provide us with a SELF ADDRESSED, STAMPED ENVELOPE at 3499 Duluth Park Lane, Ste 100 Duluth GA 30096.  We will mail you a copy of your W-2.
